For your convenience, we’ve made our new client and exotic patient forms accessible through our website. If you’re new to our practice, we recommend filling out a new client form prior to your first appointment, so you can bypass filling it out in our waiting room. If you are bringing in your exotic pet (reptile, amphibian, etc.) for the first time, please fill out the exotic patient form. This is not required for small mammals. The information you provide will help us prepare for your visit.

Please complete this form within at least one week prior of your scheduled appointment. If this form is not completed by your appointment, we will ask that you reschedule your appointment with us.
